Hypothyroidism (Underactive Thyroid)
The most important thyroid nutrition habit costs nothing. It is about timing: when you eat relative to your medication.

Hypothyroidism is one of the most commonly mismanaged conditions in Indian nutrition. The standard advice (avoid goitrogens, avoid soy, eat more iodine) is often applied too broadly and ignores the most impactful factor: the gap between your thyroid tablet and your first meal.
This guide is general nutritional information, not medical advice. If you take medication or manage a diagnosed condition, always consult your doctor before changing your diet.
The medication-food timing rule most patients are not told
Levothyroxine (Thyronorm or Eltroxin) is absorbed in the upper small intestine. Anything eaten too soon after the tablet reduces how much actually enters your system.
The minimum gap is 30 minutes. The evidence suggests 45–60 minutes is better.
Chai with milkcalcium in milk binds to levothyroxine and reduces absorption. Your morning chai should come at least 45 minutes after your tablet. This is the most common reason TSH stays unstable despite medication.
Coffee reduces absorption even more than milk. Black coffee should come at least 60 minutes after the tablet.
Calcium and iron supplements should be separated from thyroid medication by at least 4 hours.
Goitrogens: the most misunderstood part of thyroid nutrition
Goitrogens are compounds in certain foods that can interfere with thyroid hormone production in very large amounts. The most common Indian goitrogenic foods are cabbage, cauliflower, mustard greens, radish, soy, bajra, and jowar.
Goitrogens are significantly reduced by cooking. You do not need to eliminate these foods. You need to cook them rather than eat them raw, and not eat them in very large quantities every single day.
A serving of cooked gobi sabzi or methi paratha does not meaningfully interfere with thyroid function when your medication is taken correctly.
What to focus on eating
Seleniumthe thyroid has the highest selenium content of any organ in the body. Essential for converting T4 to T3 (active form). Indian sources: Brazil nuts (two per day), sunflower seeds, eggs, brown rice, and mushrooms.
Zincrequired for thyroid hormone synthesis. Found in pumpkin seeds, chickpeas, lentils, and meat.
Iodineiodised salt (which most Indian households use) covers this adequately. You do not need kelp supplements.
Ironiron deficiency impairs thyroid hormone synthesis. Common in Indian women. Spinach with lemon juice and fortified cereals help, but iron supplements must be separated from your thyroid medication by 4 hours.
Haldi and ginger in cooking reduce the chronic inflammation that worsens thyroid autoimmunity (Hashimoto's).
Managing weight with hypothyroidism
Hypothyroidism slows metabolism, which makes weight gain easy and weight loss frustratingly slow. This is not a character flaw; it is physiology. Once your TSH is in a stable optimal range, weight management becomes much more responsive to dietary changes.
Protein at every meal preserves muscle mass, which supports metabolic rate. Dal, eggs, curd, and paneer at every meal is the single most impactful dietary change for metabolism.
Do not under-eat. Very low calorie diets slow metabolism further and make thyroid management harder. Eat enough, especially at breakfast and lunch.

Asked every week
Can I eat cabbage and cauliflower with hypothyroidism?
Yes, cooked. Cooking reduces goitrogenic compounds significantly. Eating gobi sabzi a few times a week when your medication is taken correctly does not meaningfully affect thyroid function.
My TSH keeps fluctuating even though I take my tablet every day. Why?
The most common reason is inconsistent timing between the tablet and food, especially morning chai with milk. A 45–60 minute gap consistently applied often stabilises TSH without changing the dose.
Should I take selenium supplements?
Only if your doctor has confirmed deficiency. Two Brazil nuts a day provides adequate selenium for most people. Excess selenium is harmful.
Does thyroid cause hair fall?
Yes. Hair fall is common with hypothyroidism and usually improves once TSH is well-controlled. Iron deficiency alongside hypothyroidism also causes hair fall; check both.
Is bajra bad for thyroid?
Bajra contains goitrogens but is also nutritious. Eating it a few times a week as part of a balanced diet is generally fine. Do not make it a daily staple if your TSH is unstable.
